Financial Aid for Eye Care

Are There Any Financial Aid Options to Help me with my Child’s Medical Expenses?

Below are a few sources for assistance with medical expenses and obtaining health services for your child.

Bureau for Children with Medical Handicaps
Phone: (614) 466-1700

The Bureau for Children with Medical Handicaps (BCMH) is part of the Ohio Department of Health (ODH). BCMH links families of children with special health care needs to a list of quality providers. They help eligible low-income families obtain payment for their child’s health care needs.

Nationwide Children’s Hospital Financial Assistance Programs
Phone: (614) 722-2070

Nationwide Children’s Hospital is committed to offering financial assistance to people who have healthcare needs and are not able to pay for care. Patients must use all other resources, including application to the local Department of Job and Family Services (ODJFS), before financial assistance will be considered. Eligibility for assistance is based upon total gross income (how much you make before taxes), and the number of dependents in the family. People who have unusual situations may receive further consideration.

Ohio Department of Job and Family Services (ODJFS)
Ohio has three ways to apply for cash, food, and medical assistance through ODJFS:

  1. You can apply online at Benefits.Ohio.gov.
  2. You can fill out a "Request for Cash, Food and Medical Assistance" (JFS 7200) form and submit it to your county agency. You can get the form at the ODJFS website or by visiting your county agency. You can find the address and phone number of your county agency on the ODJFS website or by looking in the county government section of your phone book. Some county agencies have multiple locations so make sure to call first to find the location nearest you.
